Cholinergic neuron is a nerve cell which sends messages by using the neurotransmitter acetylcholine (ACh). Cholinergic neurons provide the primary source of acetylcholine to the cerebral cortex, and promote cortical activation during both wakefulness and rapid eye movement sleep. Cholinergic neurons originating in the nucleus basalis-septum area and are widely distributed throughout the brain, and physiological roles include learning, memory, and attention. The cholinergic system of neurons has been a main focus of research in aging and neural degradation, specifically as it relates to Alzheimer's disease. The dysfunction and loss of basal forebrain cholinergic neurons and their cortical projections are among the earliest pathological events in Alzheimer's disease.
The primary role of choline acetyltransferase (ChAT) is in synthesizing acetylcholine. It is most commonly used as a biomarker of cognitive decline, which is an important factor to monitor in numerous neurodegenerative disorders.

Acetylcholinesterase (AChE) is a key enzyme in the nervous system. It terminates nerve impulses by catalysing the hydrolysis of neurotransmitter acetylcholine. The expression of choline acetyl-transferase (ChAT) and acetylcholine esterase (AChE) are markers of all cholinergic neurons, as they are involved in the synthesis and degradation of ACh respectively. In addition, acetylcholinesterase activity can also be used as a marker for megakaryocytic cells.
