Dehydroepiandrosterone sulfate (DHEA sulfate or DHEAS), also known as androstenolone sulfate, is a naturally occurring steroid produced by the adrenal cortex. It is a metabolite of dehydroepiandrosterone (DHEA) and circulates in higher concentrations than DHEA itself. While DHEAS lacks hormonal activity, it is a significant neurosteroid and neurotrophin with important physiological functions.
DHEA, also known as androstenolone, is an endogenous steroid hormone precursor. It is abundantly present in the human body and is synthesized in the adrenal glands, gonads, and brain. DHEA plays a vital role as a metabolic intermediate in the production of androgen and estrogen sex steroids in various tissues. However, it also exhibits independent biological effects by binding to a range of nuclear and cell surface receptors. Additionally, DHEA acts as a neurosteroid and modulates neurotrophic factor receptors, making it significant in the central nervous system.
DHEA serves multiple functions in the body. As an androgen, it contributes to the androgenic effects observed during adrenarche, such as early hair growth, body odor, increased oiliness of hair and skin, and mild acne. DHEA is converted into testosterone and dihydrotestosterone in the skin and hair follicles, enhancing its androgenic potency. Conversely, individuals with complete androgen insensitivity syndrome lack functional androgen receptors, resulting in minimal body hair development. Furthermore, DHEA exhibits weak estrogenic activity and can be transformed into potent estrogens in certain tissues such as the vagina, leading to estrogenic effects. Its role as a neurosteroid and neurotrophin makes it important in the central nervous system, impacting various neurological functions.
DHEA and DHEAS are synthesized in the body through a series of enzymatic reactions. The synthesis of DHEA and DHEAS primarily occurs in the adrenal glands, although other tissues such as the gonads and brain also contribute to their production.
The biosynthesis of DHEA starts from cholesterol, which serves as the precursor molecule. Cholesterol is converted to pregnenolone through a series of enzymatic reactions involving the cytochrome P450 enzyme system. Pregnenolone is then converted to DHEA through the action of the enzyme 3β-hydroxysteroid dehydrogenase (3β-HSD). This conversion occurs in the smooth endoplasmic reticulum of adrenal cells. Once DHEA is formed, it can undergo further metabolism. In the adrenal glands, DHEA can be sulfated to form DHEAS. This sulfation process is catalyzed by the enzyme sulfotransferase. DHEAS is the most abundant circulating steroid in humans and serves as a storage form of DHEA. It exhibits immunostimulatory properties and counteracts the effects of glucocorticoids. The balance between cortisol and DHEAS levels is important as they have opposing actions. DHEA also serves as a precursor for other hormones such as androstenedione, testosterone, and estrogen.

The synthesis of DHEA and DHEAS is regulated by various factors. The release of adrenocorticotropic hormone (ACTH) from the pituitary gland stimulates the adrenal glands to produce DHEA. ACTH binds to specific receptors on adrenal cells, leading to the activation of enzymes involved in DHEA synthesis. Additionally, other hormones such as insulin-like growth factor 1 (IGF-1) and growth hormone also play a role in regulating DHEA production.
Moreover, DHEA and DHEAS levels in the blood undergo age-related changes, decreasing as individuals grow older. The higher levels of biologically inactive DHEAS serve as a reservoir that can be converted to active DHEA through the action of steroid sulfatase in the endoplasmic reticulum. The reciprocal conversion of DHEA to DHEAS makes DHEAS a reliable indicator of DHEA availability in serum.
Changes in DHEA and DHEAS levels have been associated with various diseases.
