General Description
Scl-70 antibody (anti-topoisomerase I) is an autoantibody directed against topoisomerase I, a nuclear enzyme that relaxes supercoiled DNA during replication and transcription.
Clinical significance
• Marker of systemic sclerosis (SSc), especially diffuse cutaneous SSc, with a specificity > 95 %.
• Positive in ~20–40 % of SSc patients overall, but up to 70 % in diffuse SSc.
• Associated with pulmonary fibrosis, digital ulcers, and renal crisis; predicts more severe organ involvement.
• Rare in other diseases (SLE, polymyositis) and healthy individuals, so its presence strongly supports the diagnosis of systemic sclerosis.
