Autoimmune infertility is an emerging field in reproductive medicine, encompassing a range of conditions where the immune system interferes with fertility. Autoimmune infertility occurs when the immune system mistakenly targets reproductive tissues or sperm, leading to diminished fertility or infertility. Women with autoimmune diseases have an increased risk of premature ovarian failure. In addition, autoantibodies may play an important role in fertilization and embryo implantation rates as well as placental development. This increases the risk of embryo implantation failure, repeated miscarriage, as well as increased risks during pregnancy such as pre-eclampsia and preterm birth.

Studies have shown that women with infertility have higher levels of autoantibodies, such as antithyroid, antiphospholipid, antigliadin, and antinuclear antibodies. These levels can be used to diagnose infertility and monitor treatment progress. For example, antinuclear antibodies (ANA) can reduce the chances of clinical pregnancy and implantation, and are associated with higher miscarriage rates. In women with systemic lupus erythematosus, autoantibodies like aPL and anti-oocytes can also contribute to infertility. Clinical symptoms such as repeated miscarriages and increased risk of thrombotic complications during pregnancy are associated with the presence of antiphospholipid antibodies in blood. It increases the risk of pathologies such as pre-eclampsia, RIC and preterm birth.
