Background
Smith antigen (Sm) is an uridine-rich protein present in the serum of patients with systemic lupus erythematosus (SLE), and it's a core protein of snRNP, small nuclear ribonucleoproteins. It is also one of the important symptomatrixes of SLE. The Smith antigen is not a single protein but rather a complex composed of various core proteins. The central component is U1 snRNP, which plays a crucial role in the RNA splicing process.

The Sm antigen is composed of at least nine components and is highly conserved among eukaryotes. With the exception of Sm B', all Sm antigens are expressed by the same gene and exhibit homology. All Sm proteins contain two common motifs: Sm1, consisting of 31 amino acids, and Sm2, made up of 13 amino acids, separated by 10-20 amino acids. This motif consists of a β-sheet formed by five antiparallel β-strands and a short α-helix at each N-terminus. The Sm motif encodes the core folding region of Sm proteins, facilitating their interactions. Research has shown that in the absence of snRNPs, Sm proteins can form stable secondary complexes through the antiparallel β-strands in the aforementioned β-sheet, which can further assemble into a circular structure with a diameter of 7 mm. Additionally, the Sm D antigen shares significant sequence homology with certain components, such as protamine and EBV. The 35-38 peptide sequence of EBV nuclear antigen-1 coincides with the 95-119 peptide sequence of Sm D suggesting that they can recognise one another and that molecular mimicry can assist in generating anti-Sm D autoantibodies.
The Smith antigen mainly participates in the RNA splicing process. snRNPs such as the Smith antigen recognise and bind to targeted RNA molecules, leading to intron removal and exon fusion, leading to final mature mRNA molecules. It is essential for controlling gene expression. But Smith antigen can be used for other RNA metabolic functions as well, including RNA processing and modification. In SLE patients, the immune system mistakenly identifies these autoantigens as foreign substances, leading to the production of autoantibodies against the Sm antigen. This autoimmune response can result in tissue damage and inflammation. As a result, the presence of anti-Smith antibodies is a significant marker for diagnosing SLE, and their levels are often used to assess disease activity and severity. Although these antibodies are highly specific for SLE, their sensitivity is relatively low; thus, a negative anti-Sm antibody test does not exclude the possibility of SLE. Additionally, anti-Smith antibodies are not only found in systemic lupus erythematosus but may also appear in other rare diseases, aiding in the exclusion of other autoimmune disorders.
