Neopterin, an organic compound belonging to the pteridine class of heterocyclic compounds, has emerged as an important biomarker in the field of immunology and disease research. It is synthesized by human macrophages upon stimulation with interferon-gamma (IFN-γ), a cytokine involved in the regulation of the immune response. Neopterin serves as an indicator of cellular immune system activation and plays a critical role in understanding various diseases and their progression.

Neopterin, chemically known as 2-Amino-6-(1,2,3-trihydroxypropyl)pteridin-4(1H)-one, has a molecular formula of C9H11N5O4 and a molecular weight of 253.215 g/mol. It exhibits a density of 2.02 g/cm3 and exists as a white crystalline powder. Neopterin belongs to the chemical group known as pteridines and is produced through a two-step biosynthesis process.
The biosynthesis of neopterin begins with guanosine triphosphate (GTP) as the precursor molecule. The first step involves the enzymatic activity of GTP cyclohydrolase, which opens the ribose group, leading to the formation of the intermediate compound. Subsequently, phosphatases catalyze the hydrolysis of the phosphate ester group, resulting in the formation of neopterin.
The breakdown of GTP by the enzyme GTP-cyclohydrolase I leads to the formation of neopterin, resulting in the production of 7,8-dihydroneopterin triphosphate. This compound serves as a precursor for several important molecules, including neopterin itself, tetrahydrobiopterin (a cofactor for enzymes involved in aromatic amino acid metabolism and nitric oxide synthesis), and dihydroneopterin. Among the various cell types, human monocytes/macrophages are the primary source of neopterin derivatives, including 5,6,7,8-tetrahydrobiopterin.
When the immune system is activated, IFN-γ triggers the upregulation of GTP-cyclohydrolase I and also stimulates the activity of an enzyme called indoleamine (2,3)-dioxygenase (IDO) in various cell types. IDO is involved in the breakdown of the amino acid tryptophan along the pathway leading to the synthesis of nicotinamide dinucleotide. Monitoring the levels of kynurenine (an intermediate in tryptophan catabolism) and tryptophan itself has proven to be a sensitive method for assessing the activation status of IDO and the cellular immune response, both in laboratory settings and in living organisms.

Neopterin has gained significant attention as a valuable disease marker of monocyte/macrophage axis activation in certain clinical situations. Measurement of neopterin concentrations in body fluids, including blood serum, cerebrospinal fluid (CSF), or urine, provides critical information about the extent and activity of several diseases involving activation of the cellular immune system, such as certain cancers, organ transplant rejection, autoimmune diseases, and viral infections.
