GREM1 Full Name
gremlin 1
GREM1 Introduction
Gremlin 1 (GREM1) is a secreted cysteine-knot protein and a potent antagonist of bone morphogenetic proteins (BMPs), with particularly high affinity for BMP-2, BMP-4, and BMP-7. First identified as a transcriptional target of the neural-inducing factor dHAND in Xenopus, GREM1 is now recognized as a key morphogen that patterns limb buds, kidney mesenchyme, and the intestinal crypt. In the adult, GREM1 is expressed by stromal fibroblasts and epithelial cells and is markedly upregulated during tissue repair, where it modulates BMP availability in the local niche.

BMP Antagonism and Morphogenetic Signaling
GREM1 binds BMP ligands in the extracellular space and prevents their engagement with BMP receptors, thereby keeping BMP-driven SMAD1/5/8 signaling in check. During limb development, GREM1 forms part of the feedback loop between the apical ectodermal ridge and the zone of polarizing activity, with its expression in the limb mesenchyme controlling digit number and skeletal pattern. In the kidney, GREM1 expressed by the ureteric bud antagonizes BMP-7 to maintain the metanephric mesenchyme in a progenitor state; its inactivation leads to premature nephron differentiation. In the intestinal crypt, GREM1 from stromal myofibroblasts establishes a BMP-low niche that sustains LGR5+ stem cells.
Fibrosis, Pulmonary Hypertension, and Cancer
Elevated GREM1 expression is a hallmark of idiopathic pulmonary fibrosis and of bleomycin-induced lung fibrosis, where it sustains fibroblast activation by antagonizing the anti-fibrotic effects of BMP-4. In pulmonary arterial hypertension, GREM1 derived from endothelial cells drives smooth muscle cell proliferation and vascular remodeling. GREM1 is also implicated in the hereditary mixed polyposis syndrome caused by a 40-kb upstream duplication that misexpresses GREM1 in the intestinal epithelium, predisposing carriers to multiple colorectal adenomas and carcinomas. Therapeutic monoclonal antibodies and oligonucleotides targeting GREM1 are being explored for fibrotic and oncologic indications.
