CEACAM6 Introduction
Carcinoembryonic antigen-related cell adhesion molecule 6 (CEACAM6), is a glycosylphosphatidylinositol (GPI)-anchored glycoprotein belonging to the carcinoembryonic antigen (CEA) family within the immunoglobulin superfamily. Unlike its transmembrane relative CEACAM1, CEACAM6 is attached to the outer leaflet of the plasma membrane via a GPI anchor, which influences its mobility and signaling capabilities. It is primarily expressed on the apical surface of epithelial cells in healthy tissues such as the gastrointestinal tract, respiratory epithelium, and granulocytes. However, its expression is dramatically upregulated in a wide spectrum of epithelial-derived carcinomas, making it a prominent tumor-associated antigen.

Mechanisms of Action in Cancer Progression
CEACAM6 promotes oncogenesis through multiple interconnected mechanisms. By engaging in homophilic (CEACAM6-CEACAM6) and heterophilic interactions (e.g., with CEACAM1 or other CEA family members), it interferes with the function of other adhesion molecules like E-cadherin and integrins. This disruption weakens cell-cell contacts and alters cell-matrix interactions, facilitating detachment. Furthermore, CEACAM6 overexpression activates key pro-survival and proliferative signaling pathways, including the PI3K/Akt and MAPK/ERK cascades. It also modulates the tumor microenvironment by influencing cytokine production and potentially shielding tumor cells from immune surveillance.
Clinical Significance as a Biomarker and Therapeutic Target
Due to its high and specific overexpression on malignant versus normal tissues, CEACAM6 serves as a valuable diagnostic and prognostic biomarker. Its levels in serum or tissue samples correlate with tumor stage, metastatic burden, and recurrence risk in several cancers. Therapeutically, CEACAM6 is an attractive target for novel interventions. Strategies under investigation include monoclonal antibodies for diagnostic imaging or drug delivery, antibody-drug conjugates (ADCs) to deliver cytotoxic agents directly to tumor cells, bispecific T-cell engagers, and CAR-T cell therapies. These approaches aim to leverage its tumor-restricted expression for precise anti-cancer effects.
