AIF1 Introduction
Allograft inflammatory factor 1 (AIF1), also widely recognized in neuroscience and pathology as Ionized calcium-binding adapter molecule 1 (IBA1), is a 17-kDa interferon-gamma-inducible calcium-binding protein. It is highly conserved across species and is predominantly and specifically expressed in cells of the monocyte and macrophage lineage, including resident microglia within the central nervous system (CNS). Structurally, AIF1 contains two highly conserved EF-hand calcium-binding motifs that are essential for mediating its intracellular biological activity.
AIF1 functions as a critical regulator of the actin cytoskeleton. Upon cellular activation by inflammatory signals and the subsequent intracellular calcium influx, AIF1 undergoes conformational changes that enable it to cross-link actin filaments. This calcium-dependent cytoskeletal reorganization is absolutely fundamental for the dynamic membrane ruffling, phagocytosis, and rapid migration exhibited by activated macrophages and microglia. Consequently, AIF1 is not merely a static structural marker; it is an active participant in the innate immune response, facilitating the physical movement of these cells to sites of tissue damage, infection, or inflammation to clear debris and release cytokines.

Because of its pivotal role in immune cell activation and migration, AIF1 is intimately linked to a wide spectrum of inflammatory and pathological conditions. Originally identified in rat cardiac allografts undergoing chronic immune rejection (which gave the protein its primary name), AIF1 is heavily implicated in systemic vascular diseases, including atherosclerosis and allograft vasculopathy. In these conditions, it actively promotes the migration and proliferation of vascular smooth muscle cells and invading macrophages, accelerating plaque formation. In the CNS, the robust upregulation of AIF1 (detected as IBA1) is the universal, gold-standard hallmark of microgliosis—the neuroinflammatory response seen in almost all brain and spinal cord pathologies. Accordingly, aberrant AIF1 expression and chronic microglial activation are central pathological drivers in neurodegenerative disorders such as Alzheimer's disease and Parkinson's disease, as well as in traumatic brain injury and ischemic stroke.
