Abstract
The programmed cell death 1 (PD-1) pathway represents a major immune checkpoint, which may be engaged by cells in the tumor microenvironment to overcome active T-cell immune surveillance. Pembrolizumab (Keytruda (R), MK-3475) is a potent and highly selective humanized mAb of the IgG4/kappa isotype designed to directly block the interaction between PD-1 and its ligands, PD-L1 and PD-L2. This blockade enhances the functional activity of T cells to facilitate tumor regression and ultimately immune rejection. Pembrolizumab binds to human and cynomolgus monkey PD-1 with picomolar affinity and blocks the binding of human and cynomolgus monkey PD-1 to PD-L1 and PD-L2 with comparable potency. Pembrolizumab binds both the CD and FG loops of PD-1. Pembrolizumab overcomes human and cynomolgus monkey PD-Li-mediated immune suppression in T-cell cultures by enhancing IL2 production following staphylococcal enterotoxin B stimulation of healthy donor and cancer patient cells, and IFN gamma production in human primary tumor histoculture. Ex vivo and in vitro studies with human and primate T cells show that pembrolizumab enhances antigen-specific I'-cell 1FN gamma and 1L2 production. Pembrolizumab does not mediate FcR or complement-driven effector function against PD-1-expressing cells. Pembrolizumab displays dose-dependent dearance and half-life in cynomolgus monkey pharmacoldnetic and toxicokinetic studies typical for human lgG4 antibodies. In nonhuman primate toxicology studies, no findings of toxicologic significance were observed. The preclinical data for pembrolizumab are consistent with the clinical anticancer activity and safety that has been demonstrated in human clinical trials.

Abstract
The aim of current study is to highlight the association between predisposition to multiple sclerosis (MS) and single nucleotide polymorphisms (SNPs) of interleukin-2 (IL2(-330) and IL2(+166)) and transforming growth factorbeta (TGF beta 1(+869) and TGF beta 1(+915)) genes. A total of 68 Iraqi Arab MS patients and 158 age-gender-ethnicity matched healthy subjects were enrolled in this study to determine the genotypes / alleles of IL2(-330), IL2(+166), TGF beta 1(+869) and TGF beta 1(+915) SNPs using sequence-specific primer polymerase chain reaction (SSP-PCR). Significant increased frequencies of GG genotype and G allele of IL2(+166 )SNP was observed in MS patients as compared to controls (77.9% vs. 31.7%, P = .000, OR = 7.6; 85% vs. 60%, P = .00, OR = 3.7, respectively). The frequency of TGF beta 1(+915) SNP GG genotype and G allele were also significantly higher in patients than in controls (72.1% vs. 48.7%, P = .002, OR = 2.7; 85% vs. 70%, P = .001, OR = 2.4, respectively). There was a complete linkage disequilibrium (LD) between - 330 and + 166 SNPs of IL2, whereas LD was absent from TGF beta 1 SNPs in MS patients. Additionally, TG haplotype of IL2(-330, +166 )and CG haplotype of TGF beta 1(+869, +915) showed a significant increased distribution in patients rather than in controls (37.2% vs. 21.8%, P = .000, OR = 2.2; 46.3 vs. 33.1%, P = .008, OR = 1.7, respectively). Consequently, GG genotype / G alleles and TG haplotype of IL2(-330,) (+166) and GG genotype / G alleles and CG haplotype TGF beta 1(+869, +915 )may comprise risk factors for susceptibility to MS in this sample of Iraqi population.
