Methotrexate (MTX) is a potent antimetabolite that has revolutionized the field of chemotherapy and immunosuppressive therapy. Methotrexate exerts its pharmacological effects through multiple mechanisms. As a folate antagonist, it competes with dihydrofolate reductase (DHFR), an enzyme essential for the synthesis of tetrahydrofolate (THF), a crucial cofactor in nucleotide synthesis. By inhibiting DHFR, Methotrexate disrupts the production of THF, leading to impaired DNA synthesis and cell division.

The mechanisms by which Methotrexate exerts its therapeutic effects in RA are multifaceted. In addition to inhibiting DHFR, MTX modulates purine metabolism, suppresses T cell activation and adhesion molecule expression, down-regulates B cells, increases CD95 sensitivity of activated T cells, inhibits methyltransferase activity, and acts as an anti-cytokine by interfering with the binding of IL-1β. These diverse mechanisms work in concert to alleviate inflammation and modify the immune response in RA patients.

Methotrexate is available in various formulations, including oral tablets, injectable solutions, and subcutaneous formulations. Following oral administration, Methotrexate is absorbed rapidly and undergoes hepatic metabolism. It is predominantly eliminated through renal excretion, with dosage adjustments necessary in patients with compromised renal function.
The distribution of Methotrexate in the body is primarily extracellular, and it does not readily cross the blood-brain barrier. The drug has a relatively long half-life, which necessitates careful monitoring and appropriate dosing intervals to prevent toxicity.
Accurate detection and quantification of MTX in biological samples are essential for monitoring drug levels, optimizing dosage, and assessing treatment efficacy. Several detection methods have been employed for this purpose, including immunoassays, chromatographic techniques, and mass spectrometry.
