KRT20 Introduction
KRT20, or keratin 20, is a type I intermediate filament protein that is selectively expressed in differentiated epithelial cells, particularly in the gastrointestinal tract, urothelium, and Merkel cells of the skin. As part of the keratin cytoskeleton, KRT20 pairs with type II keratins to form stable filament networks that provide mechanical support and maintain epithelial integrity. Its restricted and tissue-specific expression pattern makes KRT20 a well-established marker of epithelial differentiation, especially within intestinal and mucosal epithelia. Because keratin expression reflects both lineage and differentiation status, KRT20 is widely used in histopathology and cancer diagnostics.

Functionally, KRT20 contributes to the structural stability and functional specialization of mature epithelial cells. By integrating into intermediate filaments, it helps cells withstand mechanical stress and preserves cellular architecture during normal turnover and environmental challenge. KRT20 also participates in cytoskeletal remodeling during differentiation and may influence intracellular signaling by interacting with regulatory proteins and cellular junctions. Experimental studies suggest that altered KRT20 expression can affect epithelial barrier function and cellular polarity, linking cytoskeletal organization with tissue homeostasis and regenerative capacity.
Clinically, KRT20 has significant diagnostic and prognostic value in oncology and gastrointestinal disease. It is strongly expressed in colorectal adenocarcinoma and is commonly used, in combination with other keratins such as KRT7, to determine the tissue origin of metastatic tumors. Aberrant KRT20 expression is also observed in gastric, pancreatic, and urothelial carcinomas, where it may correlate with tumor differentiation status and disease progression. In colorectal cancer, detection of KRT20-positive cells in blood or bone marrow has been explored as a marker of minimal residual disease. These disease associations underscore KRT20's importance as a biomarker of epithelial differentiation and malignancy, supporting its continued relevance in cancer research, diagnostics, and translational medicine.
