NT-proBNP Introduction
NT-proBNP, or N-terminal prohormone of brain natriuretic peptide, plays a crucial role as a biomarker in the assessment of cardiac function and various cardiovascular conditions. It is primarily synthesized and released by the ventricles of the heart in response to increased myocardial wall stress and stretching, which occurs in conditions such as heart failure, myocardial infarction, and hypertension.

This precursor is then cleaved to form proBNP, which consists of an N-terminal fragment (NT-proBNP) and a C-terminal fragment (BNP). NT-proBNP and BNP are released into the bloodstream in equimolar amounts upon secretion. While BNP acts as a hormone, exerting its physiological effects on the cardiovascular system, NT-proBNP serves as an inactive prohormone. The biologically active BNP binds to specific receptors in various tissues, leading to vasodilation, diuresis (increased urine production), natriuresis (increased sodium excretion), and inhibition of the renin-angiotensin-aldosterone system. These effects collectively contribute to the regulation of blood pressure, fluid balance, and cardiac remodeling.

The concentration of NT-proBNP can provide valuable information about the severity and prognosis of heart failure. Elevated levels of NT-proBNP are associated with increased ventricular wall stress and impaired cardiac function. Thus, NT-proBNP serves as a reliable indicator of cardiac pathology and can aid in the diagnosis, risk stratification, and monitoring of heart failure patients. In addition to heart failure, NT-proBNP has diagnostic and prognostic value in other cardiovascular conditions. It can help assess left ventricular dysfunction, differentiate cardiac causes of dyspnea, detect myocardial ischemia, and evaluate the severity of valvular heart disease.
