CTF1 Introduction
Cardiotrophin-1 (CTF1, also widely abbreviated as CT-1) is a highly pleiotropic, 21.5 kDa cytokine that belongs to the interleukin-6 (IL-6) superfamily. Originally isolated and named for its potent ability to induce hypertrophic growth in neonatal cardiac myocytes, CTF1 is now recognized as a vital signaling molecule expressed across diverse tissues, including the heart, skeletal muscle, liver, adipose tissue, and the central nervous system. At the molecular level, CTF1 exerts its wide-ranging biological effects by binding to a specific heterodimeric cell-surface receptor complex composed of the leukemia inhibitory factor receptor beta (LIFRβ) and the ubiquitous signal-transducing subunit glycoprotein 130 (gp130).

Upon receptor engagement, CTF1 primarily triggers the activation of the Janus kinase/signal transducer and activator of transcription (JAK/STAT) pathway, specifically STAT3. Concurrently, it activates the MAPK/ERK and PI3K/AKT signaling cascades. Through these highly conserved networks, CTF1 functions as a master regulator of cell survival, stimulating cellular growth and exerting profound cytoprotective effects against oxidative stress, hypoxia, and ischemic injury. In the cardiovascular system, acute CTF1 signaling serves a protective, anti-apoptotic role during stress. However, chronic elevation of CTF1 is a major pathological driver of structural cardiac remodeling, characterized by maladaptive ventricular hypertrophy, fibrosis, and ultimately, congestive heart failure. Consequently, in modern clinical settings, elevated systemic levels of CTF1 serve as a highly sensitive and reliable circulating biomarker for diagnosing and assessing the severity of hypertensive heart disease, myocardial infarction, and heart failure.
Beyond the realm of cardiology, recent research has unveiled CTF1's indispensable role in systemic energy homeostasis and metabolism. CTF1 acts as an energy-sensing regulator that stimulates lipid oxidation, reduces adiposity, and significantly improves whole-body insulin sensitivity. Because of these distinct metabolic properties, the dysregulation of the CTF1 signaling axis is increasingly implicated in the pathogenesis of metabolic syndrome, including obesity, non-alcoholic fatty liver disease (NAFLD), and type 2 diabetes. Furthermore, capitalizing on its robust capacity to prevent apoptosis in motor and dopaminergic neurons, recombinant CTF1 and its pharmacological derivatives are currently being rigorously investigated as promising neuroprotective therapies for severe neurodegenerative disorders like amyotrophic lateral sclerosis (ALS) and Parkinson's disease.
