Approximately 50-60% of the population acquires the Polyomavirus JC (JCV) during childhood, resulting in an asymptomatic, lifelong latent infection characterized by persistent anamnestic IgG antibodies. In latently infected individuals, the virus can periodically reactivate or reinfection with different strains may occur, often leading to transient viremia or asymptomatic urinary shedding. While generally harmless, JCV reactivation is a serious concern in immunocompromised patients, as it is the established cause of the rare but severe brain infection, Progressive Multifocal Leukoencephalopathy (PML).
The presence of anti-JCV antibodies is a key risk factor for PML. Specifically, for patients on certain immunomodulatory therapies like natalizumab, monitoring anti-JCV antibody levels is crucial for stratifying PML risk, where significant increases may indicate reactivation or reinfection.
