Complement component 5 (C5) is a protein from the complement system playing an important role in inflammatory and cell killing process. C5a as a cell signaling protein can initiate an adaptive immune response and release cytokines with its ligand, C5a receptor (C5aR). The over-activation of C5a anaphylatoxin with C5aR can cause cytokine storm, endothelial inflammation and thrombosis in severe forms of COVID-19. The complement cascade as a multifunction system is crucial to innate host immunity. Over the past few years, C5a inhibition has garnered attention as a new therapeutic strategy for many inflammatory diseases. For instance, Eculizumab binds to C5 and prevents its cleavage into C5a and C5b, which blocks the formation of membrane attack complex (MAC). It has been used as a treatment for paroxysmal nocturnal hemoglobinuria (PNH) and atypical hemolytic uremic syndrome (aHUS).

Researchers also found high concentrations of C5a in COVID-19 patients with severe multisystem organ failure, pulmonary lesions, endotheliitis lesions and microthrombi symptom. Therefore, the C5a/C5aR axis blockade could make contributions to attenuating the cytokine storm and COVID-19-related organ syndromes. Thus, Vilobelimab and Eculizumab are believed to be potential therapy strategies for COVID-19 patients in severe condition. Additionally, it has been suggested that C5a with other known biomarkers can act together as a prognostic model of COVID-19 severity with an increased predictive capacity.
