The natriuretic peptides—proBNP (pro-brain natriuretic peptide), BNP (brain natriuretic peptide), and NT-proBNP (N-terminal pro-brain natriuretic peptide)—have emerged as essential biomarkers in cardiovascular diagnostics. Among these, BNP and NT-proBNP are particularly valuable due to their established utility in diagnosing and stratifying heart failure. These peptides play vital roles in modulating cardiovascular homeostasis through natriuretic, diuretic, and vasodilatory effects.

NT-proBNP is widely used in clinical practice due to its superior stability and longer half-life compared to BNP. Elevated NT-proBNP levels are strongly associated with heart failure and can be used to distinguish heart failure from other causes of dyspnea. However, there is a "gray zone" for both NT-proBNP and BNP levels in the diagnosis of HF in which sensitivity and specificity are lower and the diagnosis of HF is less certain (Table 1). Thus, this gray zone is an area where additional biomarkers are needed to assist with diagnosis.
Table 1 Rule in, rule out, and gray zone for BNP and NT-proBNP.
| Diagnosis | BNP (pg/mL) | NT-proBNP (pg/mL) | ||
| Any age | <50 years | 50–74 years | ≥75 years | |
| Rule out | <100 | <300 | <300 | <300 |
| Gray Zone | 100–500 | 300–450 | 300–900 | 300–1,800 |
| Rule in | >500 | >450 | >900 | >1,800 |
BNP/NT-proBNP can be used as predictors of the long‐term prognosis in patients with heart failure. Elevated NT-proBNP levels correlate with increased mortality, higher hospitalization rates, and worse clinical outcomes across both acute and chronic HF patients.
Serial measurements of NT-proBNP can track treatment effectiveness, with a reduction in peptide levels often correlating with clinical improvement, such as improved functional class or reduced hospitalization. On the contrary, persistently elevated levels may signal inadequate therapy or worsening disease.
