Pepsinogen is a powerful and abundant protein digestive enzyme secreted by the gastric chief cells as a proenzyme and then converted by gastric acid in the gastric lumen to the active enzyme pepsin. Pepsinogen I is secreted by the chief cells and mucus neck cells in the fundus of the stomach, and pepsinogen II throughout the stomach and proximal duodenum. A decrease in serum levels of pepsinogen I and in the I/II ratio has been shown to be a good measure of gastric atrophy.
