Chemiluminescent immunoassay (CLIA) is a quantitative detection technology that applies chemiluminescent technology to the immune recognition reaction. The basic idea is to label the chemiluminescent substance on the antigen or antibody, and after the specific binding with the corresponding antibody or antigen in the sample to form an immune complex. Then, by adding an oxidant or luminescent substrate, a luminescent reaction is induced, and a detectable light signal is generated. CLIA is a sensitive and specific laboratory method which utilizes the principle of immunochemistry with chemiluminescence. It is most frequently applied for quantitative or qualitative determination of a number of analytes including but not limited to hormones, proteins, antibodies, and drugs for use in clinical diagnostics, pharmaceutical industry, and environmental analysis. The basis of the chemiluminescent immunoassay is the immune reaction between an antigen and its corresponding antibody. One of these is labelled with a chemiluminescent reporter label, often acridinium esters, luminol or isoluminol derivatives. In the presence of the appropriate oxidizing agent, the antigen-antibody binding event is coupled to a chemical reaction generating light. The intensity of the emitted light is proportional to the amount of analyte in the sample. The chemiluminescence generated is detected with a luminometer.

CLIA offers several advantages over traditional enzyme-linked immunosorbent assays (ELISA). First, CLIA systems usually have higher sensitivity in picogram (sometimes in femtogram) level, which is sufficient for detecting very low concentrations of an analyte. Second, the chemiluminescent reaction is rapid and does not require an external light source. This reduces the background signal and improves the signal to noise ratio. Third, the CLIA platforms can be easily automated. This has made them suitable for high throughput screening (HTS) in clinical laboratories.

There are two main formats of CLIA: direct and indirect. In direct CLIA, the chemiluminescent label is directly attached to the antibody or antigen. By contrast, in indirect CLIA the label is carried by a secondary antibody, which binds to the primary antibody or antigen in the immunoassay complex. To date, CLIA has been increasingly used for the diagnosis of infectious diseases, as well as of cancer markers, cardiac markers, thyroid function, and others. The assay's speed, sensitivity and flexibility have continued to render it an important technique in the practice of modern analytical and diagnostic science.
