Copeptin (also known as CT-proAVP) is a 39-amino acid-long glycosylated peptide derived from the C-terminus of pre-pro-hormone of arginine vasopressin, neurophysin II and copeptin.

Arginine vasopressin (AVP), also known as the antidiuretic hormone (ADH), is involved in multiple cardiovascular and renal pathways and abnormal level of AVP are associated with various diseases. Reliable measurement of plasma AVP concentration is difficult and is subject to preanalytical and analytical errors. Copeptin is cosynthesized with AVP and is found in equimolar amounts with AVP in the circulation of healthy and critically ill subjects. The size and half-life of copeptin permit an easier immunological testing, compared to vasopressin, and hence copeptin is proposed as a reliable AVP surrogate.

Clinical use

Copeptin measurement has been shown to be useful in various clinical indications, including the diagnosis of diabetes insipidus and the monitoring of sepsis and cardiovascular diseases. The clinical interest in copeptin testing is closely linked to the pathophysiological pathways in which vasopressin is involved: polydipsia-polyuria syndrome, hyponatremia, syndrome of inappropriate antidiuretic hormone secretion (SIADH) as well as heart failure and acute coronary syndrome.

Clinical reference value

The concentration of copeptin in the blood circulation ranges from 1 to 12 pmol/L in healthy individuals. The levels of copeptin are slightly higher in men than in women and are not influenced by age.

Test method

Unlike AVP, copeptin is stable in EDTA plasma for up to 14 days at room temperature, while in citrate and heparin plasma copeptin is stable for 7 days. It can easily be measured ex vivo by manual or fully automated chemiluminescence assays. The assay requires minimal serum or plasma volume and the overall time for completing the analysis is between 20 and 30 minutes.
