Eculizumab is a groundbreaking therapeutic agent that has revolutionized the treatment of several life-threatening disorders, including paroxysmal nocturnal hemoglobinuria (PNH), atypical hemolytic uremic syndrome (aHUS), neuromyelitis optica, and generalized myasthenia gravis (MG). Eculizumab was the first drug approved for each of its uses. Mechanism of Action

When the complement system is dysregulated or overactivated, it can lead to tissue damage, inflammation, and the destruction of healthy cells. In certain rare blood disorders, such as PNH and aHUS, excessive complement activation contributes to the pathogenesis of the diseases.

Eculizumab acts by inhibiting the cleavage of C5 into its active forms, C5a and C5b. This prevents the generation of the membrane attack complex (MAC), which is formed by the sequential assembly of complement proteins and causes cell lysis. The MAC can insert itself into the cell membranes of target cells, leading to their destruction. This interruption of the complement cascade at the C5 level effectively inhibits downstream complement-mediated processes, including cell lysis and inflammatory responses.

By inhibiting the complement pathway, Eculizumab reduces the destruction of red blood cells in PNH, thereby alleviating anemia and other associated symptoms. In aHUS, Eculizumab prevents the formation of blood clots and subsequent kidney damage. In MG, it may help mitigate the autoimmune response by inhibiting complement-mediated inflammation at the neuromuscular junction.

Adverse effects of Eculizumab

While Eculizumab has demonstrated significant efficacy in the treatment of rare blood disorders, it is important to be aware of potential adverse effects associated with its use. Adverse effects can vary in severity and frequency among patients. It is crucial for healthcare providers to closely monitor patients receiving Eculizumab and promptly address any potential concerns. Here are some commonly reported adverse effects:

• Infections: Eculizumab may increase the risk of certain types of infections, particularly those caused by encapsulated bacteria. This is because the complement system plays a crucial role in the body's defense against these pathogens. Vaccination against Neisseria meningitidis, a bacterium associated with meningococcal infections, is recommended prior to starting Eculizumab therapy. Additionally, patients should be closely monitored for any signs of infection and promptly treated if necessary.
• Meningococcal Infections: Although rare, Eculizumab has been associated with an increased risk of meningococcal infections, including meningitis and septicemia. These infections can be life-threatening and require immediate medical attention. To mitigate this risk, patients receiving Eculizumab should adhere to appropriate vaccination schedules and receive recommended booster doses according to local guidelines.
• Infusion-Related Reactions: Some patients may experience infusion-related reactions during or shortly after receiving Eculizumab. These reactions can include symptoms such as headaches, fever, chills, nausea, vomiting, and fatigue. Most infusion-related reactions are mild to moderate in severity and can be managed by adjusting the infusion rate or administering supportive medications. However, in rare cases, severe reactions may occur, requiring immediate medical intervention.
• Thromboembolic Events: In some cases, Eculizumab treatment has been associated with an increased risk of thrombotic events, including deep vein thrombosis (DVT) and pulmonary embolism (PE). These events can be life-threatening and require prompt medical attention. Patients should be monitored for signs and symptoms of thromboembolic events, such as swelling, pain, or shortness of breath, and appropriate prophylactic measures may be considered.
• Hemolysis: While Eculizumab is primarily used to prevent hemolysis in conditions like PNH, in some rare cases, it may paradoxically induce or worsen hemolysis. This phenomenon is termed "Eculizumab-induced hemolysis" and is thought to occur due to the rapid clearance of complement-opsonized red blood cells. Any unexplained worsening of anemia or other signs of hemolysis should be evaluated promptly.
