Crimean-Congo haemorrhagic fever (CCHF) is a severe tick-borne illness caused by the CCHF virus (CCHFV). It is prevalent in Africa, the Middle East, Asia, and parts of Europe, with high fatality rates. The expanding range of the Hyalomma tick vector poses an increasing risk. While CCHFV infects various animal species, only humans experience severe illness, and subclinical infections may be more common than recognized. Currently, there are no licensed vaccines or specific antivirals available for CCHF treatment. Understanding the host and viral factors contributing to CCHFV pathogenesis remains limited.
The incubation period of CCHF ranges from three to seven days. Most cases are asymptomatic or mild, with children experiencing milder forms. Severe cases involve increased vascular permeability and a cytokine storm. Symptoms include flu-like manifestations (fever, myalgia, headache, and malaise), photophobia, diarrhea, abdominal pain, and vomiting. Severe cases may have hemorrhagic manifestations and changes in mood and behavior.
The virus enters the host through epithelial cells and spreads to the lymphatic system, infecting organs such as the liver and spleen, leading to multiorgan failure. The disease usually follows distinct phases: incubation, pre-hemorrhagic, hemorrhagic, and convalescence. The hospitalization fatality rate is around 30%. Survivors typically improve after 9-10 days, but some may have a slower recovery. Abnormalities in blood and biochemical tests are observed, including low platelet and white blood cell counts, liver enzyme elevation, and prolonged bleeding times. High viral load, severe thrombocytopenia, elevated liver enzymes, and prolonged bleeding times indicate severe disease and a higher risk of fatality.
CCHFV is a virus belonging to the Orthonairovirus genus in the Nairoviridae family. It was first identified in 1944 in Crimea and subsequently isolated in 1956 in Congo, leading to its current name. The virus has a spherical shape with a diameter of about 80-100 nm. The gene structure of CCHFV consists of three segments of single-stranded negative-sense RNA. Each segment encodes specific viral proteins essential for replication and assembly of the virus.
CCHFV strains are classified into seven genotypes (I to VII) based on the genetic sequence of the S segment. Genotype IV is further divided into subgenotypes IVf and IVg. The phylogeny of CCHFV varies among the three genome segments due to reassortment events between different genotypes.

Early and accurate diagnosis of CCHF is crucial for prompt response and control measures. In the acute phase of the disease, laboratory diagnosis involves detecting CCHFV RNA using molecular methods, as well as CCHFV antigen and virus isolation. In general, CCHFV RNA testing is effective during the first week of illness. However, the high genetic variability of CCHFV strains may reduce the efficacy of molecular testing and affect its diagnostic ability.
Metagenomic next-generation sequencing can aid pathogen identification in complex cases. Serological methods, such as ELISA or indirect immunofluorescence, detect CCHFV-specific IgM and IgG antibodies after the fifth day of illness. Confirmation of CCHFV infection relies on detecting IgM antibodies, seroconversion, or a significant increase in IgG antibody levels over time. It's important to note that severe or fatal cases may have delayed or absent antibody responses. A combination of molecular and serological approaches is recommended for an accurate diagnosis.
Currently, therapeutic options for CCHFV are limited, and most efforts have centered around inhibiting viral replication or modulating the host's response to the infection.
