General Description

Avian antibodies have been recognized for decades and offer many advantages to mammalian antibodies in research and diagnostics (Larsson, 1993). The only avian species in which antibodies are highly defined and easily accessible are the chicken. In mammals, immunoglobulins are classified into five groups: IgA, IgD, IgE, IgG and IgG. Chickens possess three principle classes of immunoglobulins: IgA, IgY (IgG) and IgM (Lebacq-Verheyden, 1974). Moreover, the structures of the corresponding immunoglobulins are significantly different, namely in the number of constant domains within the heavy chains. While avian IgY (IgG) is comprised of four constant domains, mammalian IgG contains only three (Hodek, 2003). Chicken IgG is the major serum antibody but is also actively transported to the egg in a manner similar to the placental transfer of IgG in mammals (Larsson 1993 – Review-Chicken Antibodies). The average concentration of IgY (IgG) in chicken serum (and also yolk) is 6 to 13 mg/ml. IgY (IgG) is also found mainly in the egg yoke but is also present in the egg white but in low amounts.

Abstract

Infants and young children are uniquely susceptible to primary viral and bacterial infections, predisposing them to responses of greater frequency and severity than in adults. Etiologies and manifestations of infections in pediatric patients are often different than those in adults. It can be challenging for clinical laboratories to implement appropriate microbiologic methods for rapid and accurate diagnoses in this population. Laboratorians should be cognizant of the distinctive features of children to provide comprehensive pediatric clinical microbiology services. This article discusses laboratory aspects of several clinically significant pediatric pathogens that cause severe harm to patients and impact public health responses.

AA amyloidosis associated with morbid obesity (clinical case)

Abstract

We present the case of a 45-year-old woman who was hospitalized due to severe macrocytic anemia and renal failure. The patient presented a morbid obesity. The immunological study showed anti-ENA anti-SSA (Ro52) positive, with negative antinuclear antibodies. Also in the proteinogram (serum immunofixation) the presence of monoclonal bands IgG lambda and IgG kappa, monoclonal component 7.2% (4.68 g/L), with elevation of free light chains (kappa 95.94 mg/L (3.3-19.4), evidenced, lambda 145.17 mg/L (5.71-26.3)). The bone marrow study showed an infiltration of 5% of plasma cells and positive for AA amyloid. Finally, a percutaneous renal biopsy was performed, which again showed amyloid infiltration. In the genetic study, 2 mutations of the family Mediterranean fever gene (MEFV) have been identified. Secondary AA amyloidosis has been described associated with obesity, in addition to a percentage of cases of unknown etiology.
